The PA Department of Health requirements are listed below. Failure to comply with these guidelines may result in your child being excluded from school per PA Department of Health regulations.

For attendance in all grades:

  • 4 doses of tetanus* (1 dose on or after the 4th birthday)
  • 4 doses of diphtheria* (1 dose on or after the 4th birthday)
  • 4 doses of acellular pertussis* (1 dose on or after the 4th birthday)
  • 4 doses of polio** (4th dose on or after 4th birthday and at least 6 months after previous does)
  • 2 doses of measles***
  • 2 doses of mumps***
  • 2 dose of rubella (German measles)***
  • 3 doses of hepatitis B
  • 2 doses of varicella (chickenpox) vaccine or evidence of immunity

* Usually given as DTP or DTaP or if medically advisable, DT or Td

** A 4th dose is not necessary if the third dose was administered at age 4 years or older and at least 6 months after the previous dose

***Usually given as MMR

Additional Immunization Requirements for 7th grade students:

  • 1 dose of tetanus, diphtheria, acellular pertussis (Tdap) on the first day of 7th grade
  • 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 7th grade

Additional Immunization Requirements for 12th grade students:

  • 1 dose of meningococcal conjugate vaccine (MCV) on the first day of 12th grade (unless one dose was given at age 16 or older)

Please contact your family physician regarding these changes and inquire about the status of your child’s immunizations to determine if they are current with the updated standards. Also, please provide an updated immunization record to your school nurse.
